What is Recuperative Care?

Recuperative care provides short-term medical and social services for individuals experiencing homelessness or housing insecurity who need a safe and supportive environment to recover from illness, injury, or hospitalization. These individuals may not require continued hospitalization but lack the proper environment or support for healing.
At Epic Home Healthcare LLC, we bridge the gap between hospital discharge and long-term housing or self-sufficiency, ensuring that individuals receive the necessary care to prevent avoidable hospital readmissions and provide a path toward stability.

Who We Serve

Our Recuperative Care Program is designed for vulnerable adults who:
  • Have been discharged from a hospital but lack a safe place to recover
  • Are experiencing homelessness or housing insecurity
  • Require short-term medical care and social support
  • Need assistance managing their medications, wound care, and overall recovery
  • Are at risk of hospital readmission without proper post-discharge care

How to Refer a Patient or Get Involved

We welcome hospital case managers, social workers, and community organizations to refer individuals in need of recuperative care services.

Eligible Members

To be eligible for recuperative care services, an MHCP (Minnesota Health Care Programs) member must meet the following criteria:
  • 21 years or older if they have Medical Assistance, or 19 years or older if they have MinnesotaCare
  • Experiencing homelessness or are unhoused
  • In need of short-term medical care for fewer than 60 days
  • Requiring assistance with the covered recuperative care services
